How can I arrange an appointment with the doctor?

You will need to complete a New Patient Appointment Request Form.

Can I be referred to your practice for an injection only?

Yes. Your physician must send a prescription stating exactly what injection they want done, your insurance information and any pre-authorizations/pre-certifications that are needed. The procedure will be done in our office under x-ray guidance. When completed, your follow up appointment should be made with the referring physician.

What can I expect from my first appointment?

Your consultation with the doctor is in a 1 hour time slot. They will discuss your history and present pain issues. This will be followed by a physical exam and brief discussion around a treatment plan for you.

Prior to your appointment you should complete the New Patient Questionnaire found in the Forms section of our website. If for any reason you are unable to complete the form online, we will be glad to mail you a copy. This questionnaire must be completed fully prior to your arrival.

The patient questionnaire must be filled out completely prior to your arrival.
